Diagnostics before the build used Rust's default `Error: ...` output
instead of the `bear: error:` format, and many messages stacked internal
layer names ("Failed to create consumer", "Serialization error",
"Generic IO error") around the real cause.

The format now covers every diagnostic, startup errors included, with
command-line usage errors left to the usage text. An error about a file
names it once with the action and the cause, and no internal names.
