Remove the batch_confirm autofix tier, simplifying 3-tier routing
(auto/batch_confirm/present) to 2-tier (auto/present). Items with one
clear correct fix are now auto-applied and visibly summarized rather
than requiring an interactive approval prompt that users virtually
always accepted.

Auto criteria expanded to cover both internal reconciliation and
implied additions. Negative boundary examples added to the subagent
template to prevent over-classification. Headless output updated with
per-fix detail so agent callers retain visibility.
