Trevin Chow 62359d824a docs(ce-worktree): drop changelog-in-doc FAQ; refresh stale skill-index rows (#946) ...

A user-facing doc should describe what the skill does now, not narrate its own
change history. Removes the 'What happened to .env copying and mise/direnv
trust?' FAQ entry (process exhaust — that belongs in the PR/commit, not the
durable doc). Also refreshes the skill-index rows in docs/skills/README.md and
plugins/compound-engineering/README.md, which still advertised the removed
.env-copy / dev-tool-trust behavior, to the isolation-guardrail description
(addresses Codex P3 on PR #948).
