Trevin Chow 40aacf79a5 fix(ce-code-review): fall back when a started peer never reviewed the diff ...

A cross-model peer whose CLI fails before reviewing (for example an older
claude rejecting --safe-mode) left the adversarial lens empty, because only
quota and auth failures ran the did-not-run fallback. Any diagnostic showing
the peer never reviewed now runs that fallback: one replacement peer when
eligible, otherwise the in-process adversarial-reviewer. A diagnostic that
cannot show whether the peer reviewed still only names the failure.
