- Phase 4 (renamed Close -> Handoff): replace the weak parenthetical rule with an imperative that forbids passive "ready when you are" closers; split behavior into summary-only after "Diagnosis only" vs summary plus blocking prompt after Phase 3; drop Done and Open in Proof from the menu. - Phase 2: swap the Proof option for "Diagnosis only - I'll take it from here" and anchor the "Never silently skip the question" rule right at the menu-rendering site instead of burying it in prose. - Phase 0: explicitly instruct reading the full issue conversation including the latest comments, not just the opening description - comments often contain updated repro steps, narrowed scope, or a different suspected cause.
