Extend the health check to verify the ast-grep CLI is installed and
that the ast-grep agent skill is registered. Add a Skills section to
the diagnostic output, parallel to Tools, resolved authoritatively via
`npx skills list --global --json` with a Claude Code path-probe
fallback when npx or jq is unavailable.
