Trevin Chow ac4b28f60b fix(ce-pov): name oracle peers by requested model, not "unknown model" ...

A receipt-less route records model_actual: unverified, and the reconcile
note was collapsing that into "unknown model" even though the requested
model is always known. State the rendering condition once in the panel
reference and mirror it at the reconcile points of ce-code-review and
ce-doc-review: name a peer by target and requested model, and add a
serving caveat only on a receipt mismatch or a route that requested no
model (Cursor default/Auto). The receipt policy and record fields are
unchanged.
