Trevin Chow d109a4fd95 fix(ce-proof): split pre-commit errors into token vs anchor buckets ...

Codex review flagged that the mutate() helper retried 3 codes while the
skill text listed 6 as safe-to-retry. Two of those codes (ANCHOR_NOT_FOUND,
ANCHOR_AMBIGUOUS) aren't actually blindly-retryable: they mean the quote
no longer matches content, so re-reading state doesn't help. The caller
has to tighten the anchor.

Split the pre-commit bucket:
- Token-related (STALE_BASE, BASE_TOKEN_REQUIRED, MISSING_BASE,
  INVALID_BASE_TOKEN) — same payload + fresh baseToken, helper auto-retries
- Anchor-related (ANCHOR_NOT_FOUND, ANCHOR_AMBIGUOUS) — caller must
  regenerate the quote; helper surfaces the error

Expanded the helper to handle all 4 token-related codes. Previous version
missed INVALID_BASE_TOKEN.
