Reviewer sub-agents inherited all tools from the parent session by
default, including MCP tools, Agent, Skill, and other definitions they
never use. In environments with many MCP servers or plugins, the
inherited tool definitions can exhaust Haiku's 200K context window
before the task prompt is even loaded, causing immediate "Prompt is too
long" failures.

Add explicit `tools: Read, Grep, Glob, Bash` to all 17 reviewer agents
that were missing it (7 document-review, 10 code-review). This matches
the read-only contract these agents already follow and the pattern the
other 20 code-review agents already had.
