Trevin Chow 5af3055ff9 fix(proof): replace HITL review loop with one-way publish ...

The Proof human-in-the-loop flow (write local -> upload -> annotate ->
ingest comments -> reconcile/sync back -> re-review) was slow and
clunky. Replace it with a one-way "Publish to Proof -- shareable link"
handoff across ce-plan, ce-brainstorm, and ce-ideate: ce-proof creates
a shared doc and returns a URL; the local file stays canonical and
nothing syncs back to disk.

- Remove ce-proof's HITL-review mode and references/hitl-review.md;
  reframe the skill (and docs/skills/ce-proof.md) around
  publish/read/comment/edit.
- Collapse the 4-status return state machine and post-HITL re-review in
  ce-plan/ce-brainstorm/ce-ideate to a simple publish-and-return.
- Rename the menu option to "Publish to Proof" with matching routing,
  update the four skill tests, and refresh docs/skills wording.
