Trevin Chow 829dc490d8 fix(ce-doc-review): drop HTML dedup comments from deferred findings ...

The Defer-append wrote <!-- dedup-key --> comments into the document, violating
the markdown rendering contract's no-mixed-HTML invariant and the
no-process-exhaust principle. Re-derive the dedup key from the visible entry
(section + title + a fingerprint of the rendered why_it_matters); idempotence
holds without hidden metadata. Pre-existing comments are ignored for matching
and stripped on edit.
