PHPickerViewController stayed interactive while the "Preparing Media…" overlay was shown, so tapping "Add" again called didFinishPicking a second time. Each call started its own import and completion, so the selected photos and videos were sent twice. Only handle the first selection per presentation and disable interaction with the picker once it has been accepted.
