Denis ArefevandGreg Kroah-Hartman 99cde0a7b1 block: Avoid mounting the bdev pseudo-filesystem in userspace

The bdev pseudo-filesystem is an internal kernel filesystem with which
userspace should not interfere. Unregister it so that userspace cannot
even attempt to mount it.

This fixes a bug [1] that occurs when attempting to access files,
because the system call move_mount() uses pointers declared in the
inode_operations structure, which for the bdev pseudo-filesystem
are always equal to 0. `inode->i_op = &empty_iops;`

This happens because iocg->ioc is NULL. The field is initialized by
ioc_pd_init() and never cleared. The NULL deref is caused by
blkcg_activate_policy() installing blkg_policy_data before initializing it.

blkcg_activate_policy() was doing the following:

1. Allocate pd's for all existing blkg's and install them in blkg->pd[].
2. Initialize all pd's.
3. Online all pd's.

blkcg_activate_policy() only grabs the queue_lock and may release and
re-acquire the lock as allocation may need to sleep. ioc_weight_write()
grabs blkcg->lock and iterates all its blkg's. The two can race and if
ioc_weight_write() runs during #1 or between #1 and #2, it can encounter a
pd which is not initialized yet, leading to crash.

The crash can be reproduced with the following script:

I don't see a reason why all pd's should be allocated, initialized and
onlined together. The only ordering requirement is that parent blkgs to be
initialized and onlined before children, which is guaranteed from the
walking order. Let's fix the bug by allocating, initializing and onlining pd
for each blkg and holding blkcg->lock over initialization and onlining. This
ensures that an installed blkg is always fully initialized and onlined
removing the the race window.

Ming LeiandGreg Kroah-Hartman aa7cb24019 blk-cgroup: wait for blkcg cleanup before initializing new disk

When a queue is shared across disk rebind (e.g., SCSI unbind/bind), the
previous disk's blkcg state is cleaned up asynchronously via
disk_release() -> blkcg_exit_disk(). If the new disk's blkcg_init_disk()
runs before that cleanup finishes, we may overwrite q->root_blkg while
the old one is still alive, and radix_tree_insert() in blkg_create()
fails with -EEXIST because the old blkg entries still occupy the same
queue id slot in blkcg->blkg_tree. This causes the sd probe to fail
with -ENOMEM.

Fix it by waiting in blkcg_init_disk() for root_blkg to become NULL,
which indicates the previous disk's blkcg cleanup has completed.

successful but the function later encounters an error, we need
to clean up the blk_mq_sysfs resources.

Waiman LongandGreg Kroah-Hartman 892faa76be blk-cgroup: Reinit blkg_iostat_set after clearing in blkcg_reset_stats()

When blkg_alloc() is called to allocate a blkcg_gq structure
with the associated blkg_iostat_set's, there are 2 fields within
blkg_iostat_set that requires proper initialization - blkg & sync.
The former field was introduced by commit 3b8cc62987 ("blk-cgroup:
Optimize blkcg_rstat_flush()") while the later one was introduced by
commit f733164829 ("blk-cgroup: reimplement basic IO stats using
cgroup rstat").

Unfortunately those fields in the blkg_iostat_set's are not properly
re-initialized when they are cleared in v1's blkcg_reset_stats(). This
can lead to a kernel panic due to NULL pointer access of the blkg
pointer. The missing initialization of sync is less problematic and
can be a problem in a debug kernel due to missing lockdep initialization.

Fix these problems by re-initializing them after memory clearing.

Commit 7b815817aa ("blk-mq: add helper for checking if one CPU is mapped to specified hctx")
needs to check queue mapping via tag set in hctx's cpuhp handler.

However, q->tag_set may not be setup yet when the cpuhp handler is
enabled, then kernel oops is triggered.

Fix the issue by setup queue tag_set before initializing hctx.

Commit a46c27026d ("blk-mq: don't schedule block kworker on isolated CPUs")
rules out isolated CPUs from hctx->cpumask, and hctx->cpumask should only be
used for scheduling kworker.

Add helper blk_mq_cpu_mapped_to_hctx() and apply it into cpuhp handlers.

This patch avoids to forget clearing INACTIVE of hctx state in case that one
isolated CPU becomes online, and fixes hang issue when allocating request
from this hctx's tags.

Ming LeiandGreg Kroah-Hartman b20159ef47 blk-mq: don't schedule block kworker on isolated CPUs

Kernel parameter of `isolcpus=` or 'nohz_full=' are used to isolate CPUs
for specific task, and it isn't expected to let block IO disturb these CPUs.
blk-mq kworker shouldn't be scheduled on isolated CPUs. Also if isolated
CPUs is run for blk-mq kworker, long block IO latency can be caused.

Kernel workqueue only respects CPU isolation for WQ_UNBOUND, for bound
WQ, the responsibility is on user because CPU is specified as WQ API
parameter, such as mod_delayed_work_on(cpu), queue_delayed_work_on(cpu)
and queue_work_on(cpu).

So not run blk-mq kworker on isolated CPUs by removing isolated CPUs
from hctx->cpumask. Meantime use queue map to check if all CPUs in this
hw queue are offline instead of hctx->cpumask, this way can avoid any
cost in fast IO code path, and is safe since hctx->cpumask are only
used in the two cases.

loop devices under heavy stress-ng loop streessor can trigger many
capacity change events in a short time. Each event prints an info
message from set_capacity_and_notify(), flooding the console and
contributing to soft lockups on slow consoles.

Switch the printk in set_capacity_and_notify() to
pr_info_ratelimited() so frequent capacity changes do not spam
the log while still reporting occasional changes.

During system suspend, wakeup capable IRQs for block device can be
delayed, which can cause blk_mq_hctx_notify_offline() to hang
indefinitely while waiting for pending request to complete.
Skip the request waiting loop and abort suspend when wakeup events are
pending to prevent the deadlock.

Darrick J. WongandGreg Kroah-Hartman 311427c938 block: fix race between set_blocksize and read paths
commit c0e473a0d2 upstream.

With the new large sector size support, it's now the case that
set_blocksize can change i_blksize and the folio order in a manner that
conflicts with a concurrent reader and causes a kernel crash.

Specifically, let's say that udev-worker calls libblkid to detect the
labels on a block device.  The read call can create an order-0 folio to
read the first 4096 bytes from the disk.  But then udev is preempted.

Next, someone tries to mount an 8k-sectorsize filesystem from the same
block device.  The filesystem calls set_blksize, which sets i_blksize to
8192 and the minimum folio order to 1.

Now udev resumes, still holding the order-0 folio it allocated.  It then
tries to schedule a read bio and do_mpage_readahead tries to create
bufferheads for the folio.  Unfortunately, blocks_per_folio == 0 because
the page size is 4096 but the blocksize is 8192 so no bufferheads are
attached and the bh walk never sets bdev.  We then submit the bio with a
NULL block device and crash.

Therefore, truncate the page cache after flushing but before updating
i_blksize.  However, that's not enough -- we also need to lock out file
IO and page faults during the update.  Take both the i_rwsem and the
invalidate_lock in exclusive mode for invalidations, and in shared mode
for read/write operations.

I don't know if this is the correct fix, but xfs/259 found it.

Change the 'ret' variable in blk_stack_limits() from unsigned int to int,
as it needs to store negative value -1.

Storing the negative error codes in unsigned type, or performing equality
comparisons (e.g., ret == -1), doesn't cause an issue at runtime [1] but
can be confusing.  Additionally, assigning negative error codes to unsigned
type may trigger a GCC warning when the -Wsign-conversion flag is enabled.

No effect on runtime.

the maximum buffer size to BIO_MAX_INLINE_VECS.

Replace the UIO_MAXIOV symbolic name with the more contextually
appropriate BIO_MAX_INLINE_VECS.

A new field 'online' is added to blkg_policy_data to fix following
2 problem:

1) In blkcg_activate_policy(), if pd_alloc_fn() with 'GFP_NOWAIT'
   failed, 'queue_lock' will be dropped and pd_alloc_fn() will try again
   without 'GFP_NOWAIT'. In the meantime, remove cgroup can race with
   it, and pd_offline_fn() will be called without pd_init_fn() and
   pd_online_fn(). This way null-ptr-deference can be triggered.

2) In order to synchronize pd_free_fn() from blkg_free_workfn() and
   blkcg_deactivate_policy(), 'list_del_init(&blkg->q_node)' will be
   delayed to blkg_free_workfn(), hence pd_offline_fn() can be called
   first in blkg_destroy(), and then blkcg_deactivate_policy() will
   call it again, we must prevent it.

The new field 'online' will be set after pd_online_fn() and will be
cleared after pd_offline_fn(), in the meantime pd_offline_fn() will only
be called if 'online' is set.

Commit eca0025faa ("block, bfq: split sync bfq_queues on a
per-actuator basis"), which is a backport of 9778369a2d ("block,
bfq: split sync bfq_queues on a per-actuator basis") re-introduces UAF
bug originally fixed by b600de2d7d ("block, bfq: fix uaf for bfqq in
bic_set_bfqq()") and backported to 6.1 in cb1876fc33 ("block, bfq:
fix uaf for bfqq in bic_set_bfqq()").

bfq_release_process_ref() may release the sync_bfqq variable, which
points to the same bfqq as bic->bfqq member for call context from
__bfq_bic_change_cgroup(). bic_set_bfqq() then accesses bic->bfqq member
which leads to the UAF condition.

Fix this by bringing the incriminated function calls back in correct
order.

Olivier GayotandGreg Kroah-Hartman 30f67c1048 block: fix conversion of GPT partition name to 7-bit
commit e06472bab2 upstream.

The utf16_le_to_7bit function claims to, naively, convert a UTF-16
string to a 7-bit ASCII string. By naively, we mean that it:
 * drops the first byte of every character in the original UTF-16 string
 * checks if all characters are printable, and otherwise replaces them
   by exclamation mark "!".

This means that theoretically, all characters outside the 7-bit ASCII
range should be replaced by another character. Examples:

 * lower-case alpha (ɒ) 0x0252 becomes 0x52 (R)
 * ligature OE (œ) 0x0153 becomes 0x53 (S)
 * hangul letter pieup (ㅂ) 0x3142 becomes 0x42 (B)
 * upper-case gamma (Ɣ) 0x0194 becomes 0x94 (not printable) so gets
   replaced by "!"

The result of this conversion for the GPT partition name is passed to
user-space as PARTNAME via udev, which is confusing and feels questionable.

However, there is a flaw in the conversion function itself. By dropping
one byte of each character and using isprint() to check if the remaining
byte corresponds to a printable character, we do not actually guarantee
that the resulting character is 7-bit ASCII.

This happens because we pass 8-bit characters to isprint(), which
in the kernel returns 1 for many values > 0x7f - as defined in ctype.c.

This results in many values which should be replaced by "!" to be kept
as-is, despite not being valid 7-bit ASCII. Examples:

 * e with acute accent (é) 0x00E9 becomes 0xE9 - kept as-is because
   isprint(0xE9) returns 1.
 * euro sign (€) 0x20AC becomes 0xAC - kept as-is because isprint(0xAC)
   returns 1.

This way has broken pyudev utility[1], fixes it by using a mask of 7 bits
instead of 8 bits before calling isprint.

Single-LUN multi-actuator SCSI drives, as well as all multi-actuator
SATA drives appear as a single device to the I/O subsystem [1].  Yet
they address commands to different actuators internally, as a function
of Logical Block Addressing (LBAs). A given sector is reachable by
only one of the actuators. For example, Seagate’s Serial Advanced
Technology Attachment (SATA) version contains two actuators and maps
the lower half of the SATA LBA space to the lower actuator and the
upper half to the upper actuator.

Evidently, to fully utilize actuators, no actuator must be left idle
or underutilized while there is pending I/O for it. The block layer
must somehow control the load of each actuator individually. This
commit lays the ground for allowing BFQ to provide such a per-actuator
control.

BFQ associates an I/O-request sync bfq_queue with each process doing
synchronous I/O, or with a group of processes, in case of queue
merging. Then BFQ serves one bfq_queue at a time. While in service, a
bfq_queue is emptied in request-position order. Yet the same process,
or group of processes, may generate I/O for different actuators. In
this case, different streams of I/O (each for a different actuator)
get all inserted into the same sync bfq_queue. So there is basically
no individual control on when each stream is served, i.e., on when the
I/O requests of the stream are picked from the bfq_queue and
dispatched to the drive.

This commit enables BFQ to control the service of each actuator
individually for synchronous I/O, by simply splitting each sync
bfq_queue into N queues, one for each actuator. In other words, a sync
bfq_queue is now associated to a pair (process, actuator). As a
consequence of this split, the per-queue proportional-share policy
implemented by BFQ will guarantee that the sync I/O generated for each
actuator, by each process, receives its fair share of service.

This is just a preparatory patch. If the I/O of the same process
happens to be sent to different queues, then each of these queues may
undergo queue merging. To handle this event, the bfq_io_cq data
structure must be properly extended. In addition, stable merging must
be disabled to avoid loss of control on individual actuators. Finally,
also async queues must be split. These issues are described in detail

per-process bfq_queues are provided, the I/O of each process or group
of processes is still sent to only one queue, regardless of the
actuator the I/O is for. The forwarding to distinct bfq_queues will be
enabled after addressing the above issues.

Fix several issues in partition probing:

 - The bailout for a bad partoffset must use put_dev_sector(), since the
   preceding read_part_sector() succeeded.
 - If the partition table claims a silly sector size like 0xfff bytes
   (which results in partition table entries straddling sector boundaries),
   bail out instead of accessing out-of-bounds memory.
 - We must not assume that the partition table contains proper NUL
   termination - use strnlen() and strncmp() instead of strlen() and
   strcmp().

blkdev_read_iter() has a few odd checks, like gating the position and
count adjustment on whether or not the result is bigger-than-or-equal to
zero (where bigger than makes more sense), and not checking the return
value of blkdev_direct_IO() before doing an iov_iter_revert(). The
latter can lead to attempting to revert with a negative value, which
when passed to iov_iter_revert() as an unsigned value will lead to
throwing a WARN_ON() because unroll is bigger than MAX_RW_COUNT.

Be sane and don't revert for -EIOCBQUEUED, like what is done in other
spots.

class_dev_iter_(init|next)(), but does not end iterating with

Fix by ending the iterating with class_dev_iter_exit().

Before commit e418de3abc ("block: switch gendisk lookup to a simple
xarray"), lookup_gendisk will first use base_probe to load module loop,
and then the retry will call loop_probe to prepare the loop disk. Finally
open for this disk will success. However, after this commit, we lose the
retry logic, and open will fail with ENXIO. Block device autoloading is
deprecated and will be removed soon, but maybe we should keep open success
until we really remove it. So, give a retry to fix it.

atomic mode after del_gendisk"), hence for disk like scsi, following
blk_mq_destroy_queue() will not clear flush rq from tags->rqs[] as well,
cause following uaf that is found by our syzkaller for v6.6:

Other than blk_mq_clear_flush_rq_mapping(), the flag is only used in
blk_register_queue() from initialization path, hence it's safe not to
clear the flag in del_gendisk. And since QUEUE_FLAG_REGISTERED already
make sure that queue should only be registered once, there is no need
to test the flag as well.

commit 1ba0403ac6 ("block, bfq: fix uaf for accessing waker_bfqq after
splitting") fix the problem that if waker_bfqq is in the merge chain,
and current is the only procress, waker_bfqq can be freed from
bfq_split_bfqq(). However, the case that waker_bfqq is not in the merge
chain is missed, and if the procress reference of waker_bfqq is 0,
waker_bfqq can be freed as well.

Fix the problem by checking procress reference if waker_bfqq is not in
the merge_chain.

__propagate_weights() is called with an active value of zero in
ioc_check_iocgs(), which results in the high value being less than the
low value, which is undefined because the value returned depends on the
order of the comparisons.

The purpose of this expression is to ensure inuse is not more than
active and at least 1. This could be written more simply with a ternary
expression that uses min(inuse, active) as the condition so that the
value of that condition can be used if it is not zero and one if it is.
Do this conversion to resolve the error and add a comment to deter
people from turning this back into clamp().

Note that the UAF is not easy to trigger as the free path is indirected
behind a couple RCU grace periods and a work item execution. I could only
trigger it with artifical msleep() injected in blkcg_unpin_online().

Both scenarios are similar, the full memory barrier should be inserted
between 1) and 2), as well as between 3) and 4) to make sure that either
CPU0 sees BLK_MQ_S_STOPPED is cleared or CPU1 sees dispatch list.
Otherwise, either CPU will not rerun the hardware queue causing
starvation of the request.

The easy way to fix it is to add the essential full memory barrier into
helper of blk_mq_hctx_stopped(). In order to not affect the fast path
(hardware queue is not stopped most of the time), we only insert the
barrier into the slow path. Actually, only slow path needs to care about
missing of dispatching the request to the low-level device driver.

blk_rq_map_user_bvec contains a check bytes + bv->bv_len > nr_iter which
causes unnecessary failures in NVMe passthrough I/O, reproducible as
follows:

- register a 2 page, page-aligned buffer against a ring
- use that buffer to do a 1 page io_uring NVMe passthrough read

The second (i = 1) iteration of the loop in blk_rq_map_user_bvec will
then have nr_iter == 1 page, bytes == 1 page, bv->bv_len == 1 page, so
the check bytes + bv->bv_len > nr_iter will succeed, causing the I/O to
fail. This failure is unnecessary, as when the check succeeds, it means
we've checked the entire buffer that will be used by the request - i.e.
blk_rq_map_user_bvec should complete successfully. Therefore, terminate
the loop early and return successfully when the check bytes + bv->bv_len
> nr_iter succeeds.

While we're at it, also remove the check that all segments in the bvec
are single-page. While this seems to be true for all users of the
function, it doesn't appear to be required anywhere downstream.

So rq_qos_wake_function() calls wake_up_process(data->task), which calls
try_to_wake_up(), which faults in raw_spin_lock_irqsave(&p->pi_lock).

p comes from data->task, and data comes from the waitqueue entry, which
is stored on the waiter's stack in rq_qos_wait(). Analyzing the core
dump with drgn, I found that the waiter had already woken up and moved
on to a completely unrelated code path, clobbering what was previously
data->task. Meanwhile, the waker was passing the clobbered garbage in
data->task to wake_up_process(), leading to the crash.

What's happening is that in between rq_qos_wake_function() deleting the
waitqueue entry and calling wake_up_process(), rq_qos_wait() is finding
that it already got a token and returning. The race looks like this:

Normally, finish_wait() is supposed to synchronize against the waker.
But, as noted above, it is returning immediately because the waitqueue
entry has already been removed from the waitqueue.

The bug is that rq_qos_wake_function() is accessing the waitqueue entry
AFTER deleting it. Note that autoremove_wake_function() wakes the waiter
and THEN deletes the waitqueue entry, which is the proper order.

Fix it by swapping the order. We also need to use
list_del_init_careful() to match the list_empty_careful() in
finish_wait().
