Support the most common fields used by:
- KeePass 2.47 itself
- KeeWeb
- KeeTrayTOTP, and Tray TOTP

Support the most common settings:
- TOTP, with 30s period and 6 digits
- Steam Guard TOTP (30s period, custom alphabet) as supported by
  pass-otp and Android Password Store
