Kazuki YamadaandClaude Opus 5.5 226678169b feat(mcp): Migrate to MCP TypeScript SDK v2 ...

Replace @modelcontextprotocol/sdk (v1) with @modelcontextprotocol/server (v2).
The official v1-to-v2 codemod rewrote the import paths and converted the one
remaining variadic mcpServer.prompt() call to registerPrompt().

intent(mcp): Move off the v1 single-package SDK now that v2 has had several releases (2.0.0 through 2.4.0)
decision(deps): Pin ^2.3.1 rather than 2.4.0, which is still inside the 7-day npm release cooldown
decision(tests): Add a shared structuredContentOf helper because v2 types structuredContent loosely
rejected(mcp): Rewriting tool schemas for the codemod's 8 "raw shape" warnings, all false positives since every schema is already z.object
constraint(mcp): Client-visible output changes only in metadata: tool schemas declare JSON Schema 2020-12 instead of draft-07, and the experimental execution.taskSupport field is gone
