* Add LSP types and server stubs for call hierarchy support

* Update CMakeLists.txt for new LSP types

* Call hierarchy misc fixes

* Minor PositionRangeArray fixes to simplify codable conformance

* Add CodingTests for PositionRangeArray and CallHierarchy encoding
