- Add prepareCallHierarchy handler

- Use proper symbol kind in prepareCallHierarchy
- Stub out implementation of outgoingCalls

- Extract call hierarchy item data parser
- Implement incomingCalls
- Add Call Hierarchy to the list of supported features
- Resolve caller/callee definition locations for call hierarchy
