Hamish Knight 2b0e39fb85 [Sema] Add some missing null checks for getExtendedNominal ...

Make sure we handle cases where an extension couldn't resolve its
nominal. This wasn't an extensive audit, but at least fixes all of the
cases we have crasher test cases for.
