Hiroshi Yamauchi f9cd774649 [IRGen] Use symbolic references for protocols in reflection records (#92714) ...

Reflection records named protocols with their textual mangled name,
which for a private protocol includes a private discriminator. Remote
Mirror can't recover the discriminator from the protocol descriptor, so
it failed to find that protocol's field descriptor, causing leaks to
treat their weak fields as conservative references. This also fixes the
same failure for associated type descriptors of conformances to private
protocols.
