* Fix another use-after-free in SILCombine

swift::endLifetimeAtFrontier also needs to use
swift::emitDestroyOperation and delete instructions via callbacks that
can correctly remove it from the worklist that SILCombine maintains

* Add test for use-after-free in SILCombine
