Yasuhiro Matsumoto 8f422b67ea Cache executable checks during kakehashi initialization ...

Share dependency and server executable checks across filetypes during one
kakehashi initialization scan. Keep the cache local to that scan so later
installation or removal of commands is detected on the next scan.

Add regression coverage for successful and missing commands, cache lifetime,
filetype selection, disabled servers, and command overrides.
