The house rule already asked for a reason on each lint suppression,
but only review enforced it. Turning on
clippy::allow_attributes_without_reason makes clippy check it, so the
rule now uses the reason = "..." form the lint reads.

The lint is set once in [workspace.lints] and every member opts in, so
the next lints can be added in one place.

The integration fixtures carried ~75 identical dead_code and
unused_imports allows. They share one cause, tests gated on host tools,
so a single allow on the fixtures module replaces them. Three allows
in semantic and compilers-codegen were stale and are removed.
