feat(report-bug): rename to report-bug-ce and make platform-agnostic

- Rename skill to report-bug-ce to avoid name collisions with other plugins
- Replace Claude Code-specific env detection with multi-platform guidance
- Add Agent Platform/Agent Version fields to bug report template
- List platform equivalents for user interaction tools
- Fix nested code fence that broke markdown rendering
