- Validate, commit, and reply steps read per-item results from inline fixes as
  well as fixer returns, so an inline fix is never treated as an empty change.
- Dispatch rule states precedence: fixers for a real parallel batch on disjoint
  files or a fix reaching beyond what was read; everything else inline.
