[LookupVisibleDecls] Implement shadowing for unqualified lookups

Tweaked usable check:
  * Local type/func decls are usable even before declaration
  * Outer nominal Instance member are not usable
  * Type context cannot close over values in outer type contexts

Added shadowing rule by the base name:
