66b9d21000 [Runtime] Remove all use of read/write locks.
Read/write locks are not as good as you'd think; a simple mutex is better in
almost all cases.

0cf687aa2b [Build][Runtime] Replace SWIFT_STDLIB_SINGLE_THREADED_RUNTIME.
SWIFT_STDLIB_SINGLE_THREADED_RUNTIME is too much of a blunt instrument here.
It covers both the Concurrency runtime and the rest of the runtime, but we'd
like to be able to have e.g. a single-threaded Concurrency runtime while
the rest of the runtime is still thread safe (for instance).

So: rename it to SWIFT_STDLIB_SINGLE_THREADED_CONCURRENCY and make it just
control the Concurrency runtime, then add a SWIFT_STDLIB_THREADING_PACKAGE
setting at the CMake/build-script level, which defines
SWIFT_STDLIB_THREADING_xxx where xxx depends on the chosen threading package.

This is especially useful on systems where there may be a choice of threading
package that you could use.

63a09007a1 [Threading] Create new threading library and use it.
Moved all the threading code to one place.  Added explicit support for
Darwin, Linux, Pthreads, C11 threads and Win32 threads, including new
implementations of Once for Linux, Pthreads, C11 and Win32.

709359cfab Rename duplicated swift::fatalError definition in swiftRuntime and swift_Concurrency
Both swiftRuntime and swift_Concurrency had swift::fatalError
implementation, but it causes symbol conflict when -static-stdlib.

This patch renames one of the impl in swift_Concurrency to avoid
conflict.

dd6c235a2d [Runtime] Use os_unfair_lock for Mutex and StaticMutex on Darwin.
os_unfair_lock is much smaller than pthread_mutex_t (4 bytes versus 64) and a bit faster.

However, it doesn't support condition variables. Most of our uses of Mutex don't use condition variables, but a few do. Introduce ConditionMutex and StaticConditionMutex, which allow condition variables and continue to use pthread_mutex_t.

On all other platforms, we continue to use the same backing mutex type for both Mutex and ConditionMutex.

d9ba70a595 runtime: tweak the compilation
Always include all the sources as we cannot include object libraries to be
subsumed.  This allows us to conditionally build the swift runtime for foreign
hosts simultaneously (e.g. cross compile for Linux and Windows simultaneously).

a0452be947 [runtime] enhanced and refactored recently added Mutex abstraction
- added read / write lock support
- added non-fatal error support to allow use of mutex in fatal error reporting pathway
- isolated pthread implementation to it own header/cpp file pair
- expanded unit tests to cover new code as well as better test existing mutex
- removed a layer of complexity that added no real value
