[Mangling] Adjust subscript mangling to not include "subscript"
...
Change the mangling of accessors to have a variable or subscript node
as their only child node, while subscript nodes no longer contain a decl
name.

[index] Remove import.sourcefile kind from indexing
...
We don't support importing source files in source code, and they only
showed up in the index under failure conditions.

[sourcekit] Merge SourceKit into the Swift repo.
...
The code goes into its own sub-tree under 'tools' but tests go under 'test',
so that running 'check-swift' will also run all the SourceKit tests.
SourceKit is disabled on non-darwin platforms.
