Slava Pestov ebb1198d57 AST: There's no longer any reason to pass SubstitutionMap by const reference ...

SubstitutionMaps are now just a trivial pointer-sized value, so
pass them by value instead.

I did have to move a couple of functors from Type.h to SubstitutionMap.h
to resolve some issues with forward declarations.
